HSS is able to assist anyone with Support Coordination in their NDIS Plan.
However, we predominantly work with complex clients withe dual and multiple disabilities.
We have particular expertise and experience in following disabilities.

Support Coordination

We offer two ‘levels’ of Support Coordination to clients that have Support Coordination included in their NDIS Plan in the Capacity Building Section.
There can be a combination of both levels in one NDIS Plan.

Level 2 – Coordination of Supports

Level 3 – Specialised Support Coordination

Support Coordinators can help you with the following examples

  • Find and connect with local NDIS supports
  • Understand your NDIS plan
  • Understand your budget
  • Work out the best way to spend your funds
  • Request and organise reports from your Allied Health Professionals
  • Prepare for your NDIS plan review.
  • Be an active member in your community
  • Link to other government and non – government agencies
  • Link to employment and volunteering services

Think outside the Box

We can help you get the best out of your NDIS plan.
Your Support Coordinator will seek to really understand your goals and will spend the time with you on what support you need to achieve them.
These may include paid services, mainstream activities available to anyone in the community, and identifying friends and family who can be a part of your support system.

We believe in empowering people to take control of their lives, and this means we will help you achieve your goals and confidence to manage your own plan in the future.
